HOW TO SCHEDULE YOUR APPOINTMENT

1. Obtain a referral. Please ask your Physician or Nurse Practitioner to provide a referral to our office.

 

2. Verify your insurance coverage. Please contact your insurance company before your appointment to confirm that your nutrition counseling session is covered under your plan.

 

3. Schedule your appointment. You can schedule your appointment in either of the following ways: Call 571-293-1087 and leave a voicemail for the Dietitian. Book online through our website using the “Book Now” option.

 

4. For a free consultation with the Dietitian, call 571-293-1087.
